CUSD 6th & 7th Grade Survey Information

Each year we send out a climate survey issued by the state of California called California Healthy Kids Survey (CHKS).  Your participation is voluntary.  It is also very important as the information you provide will help guide our district and our school efforts to continue promoting safety, enhance learning supports, and improve students achievement. There is a parent survey, a 6th grade Opt-in survey, and a 7th grade Opt-out survey.

Please note:

• This is an anonymous survey
• All questions apply to Aviara Oaks Middle School Only
• The results are for the use of Carlsbad Unified School District and Aviara Oaks Middle School

Instructions:

• If you have more than one child at AOMS, you will only need to complete the survey once, using your oldest child as a reference.
